We have no idea when we will be re-credentialed, though we hope soon, so we can serve new patients with MA coverage!)

After filling out the Child Registration Form, press submit to securely transmit to us. Our admin team will reach out via email to confirm its receipt. From there we will verify benefits and help secure an order to evaluate and treat your child from their primary care physician. Do not hesitate to contact us with any questions along the way.
